Ummm.. IIRC it was drivers licence, passport number if you had one and medicare number. To verify your card you can just scan it and email/fax it to them. The Drivers licence, passport number and medicare number are all done over a browser based application. Quick and easy.

Yeah, that's what I thought at first, but after looking at it, I realized it wouldn't take very long. Ended up taking me no more than 10 mins. IIRC, TAB online account doesn't require veryfing your identity or card.
